- Remote | WFH We are seeking a compassionate and dedicated Mental Health Practitioner to join our Assertive Community Treatment (ACT) team. In this role, you will provide comprehensive mental health support to individuals living with serious mental illness in their homes and communities.

What you'll do:

Conduct comprehensive assessments and develop individualized treatment plans.
Provide direct clinical services, including individual therapy, medication management support, and crisis intervention.
Assist clients with accessing and maintaining housing, employment, and other essential resources.
Collaborate with a multidisciplinary team to provide integrated care and support.
Advocate for clients' rights and needs within the community.
Participate in on-call rotation and provide crisis intervention services as needed.

Qualifications:

Bachelor's or Master's Degree in Behavioral Sciences or a related field.
2,000 hours of supervised experience working with individuals with Mental Illness.
